Úloha apoptózy v ovlivnění rozsahuendometriózy a její klinickésymptomatologie

Cíl studie:
Vyšetření přítomnosti a intenzity apoptózy v ložiscích peritoneální endometriózy. Posouzeníúlohy rozdílné intenzity apoptózy v ovlivnění progrese endometriózy a manifestace subjektivníchobtíží.Typ studie: Prospektivní studie.Název a sídlo pracoviště: Gynekologicko-porodnická klinika a Histologický a embryologický ústav,1. LF Univerzity Karlovy a VFN v Praze.Metodika:U žen s endometriózou I. až III. stupně byla laparoskopicky odebrána ložiska peritoneálníendometriózy. Vzorky byly fixovány v Karnovského směsi. Přítomnost apoptózy byla detekovánaimunohistochemicky.Výsledky: Ložiska peritoneální endometriózy byla odebrána u 48 žen. Celkem 29 bylo symptomatických,19 nemělo žádné obtíže. Průměrná doba obtíží byla 16,6 měsíců. Ze 35 hodnocených vzorkůbyla přítomnost apoptózy prokázána v 11 případech. Mezi oběma skupinami s přítomnou a s nepřítomnouapoptózou nebyly zjištěny rozdíly v zastoupení jednotlivých stupňů endometriózy, v zastoupeníasymptomatických případů, ani v četnosti jednotlivých subjektivních obtíží.Závěr: Studie potvrdila častou přítomnost apoptózy v ložiscích peritoneální endometriózy. Nebylavšak zjištěna souvislost mezi přítomností apoptózy a rozsahem endometriózy nebo manifestací čicharakterem subjektivních obtíží.

Objective:
To evaluate the presence and intensity of apoptosis in lesions of peritoneal endometriosis.To consider the role of different intensity of apoptosis in the progression of the disease and in themanifestation of clinical symptoms.Design: Prospective study.Setting: Department of Obstetrics and Gynecology and Department of Histology and Embryology,1st Medical Faculty, Charles University and General Faculty Hospital, Prague.Methods: Lesions of peritoneal endometriosis were excised at laparoscopy in women with 1st to 3rdstage of the disease. Specimens were fixed in the Karnowski solution. The presence of apoptosis wasassessed immunohistochemically.Results: Biopsies of peritoneal endometriosis were consecutively taken in 48 women. Altogether, 29patients were symptomatic, and 19 did not complain of any symptoms. The average duration ofsymptoms was 16.6 months. The presence of apoptosis was detected in 11 out of 35 evaluatedspecimens. In comparing the groups with and without apoptosis, no differences were found in thestage of the disease, in the proportion of asymptomatic patients, and in the manifestation of differentsubjective complaints.Conclusions: The study confirmed a frequent presence of apoptosis in biopsies of peritoneal endometriosis.However, there were no relationships found between the presence of apoptosis and thestage of the disease or manifestation or character of subjective complaints.
